Does Sentio make up tasks that I did not say?
It is built not to. The instructions Sentio gives its AI say to include only actual tasks and not general thoughts, and a note with no tasks should produce an empty list. But an AI summarizer can still misread, reword or over-include, and a few of Sentio's conveniences, like a default reminder date, are not things you said. Check every list against the transcript before you act on it.

What the instructions say
The action-item prompts tell the model to include only actual tasks, not general thoughts, to keep deadlines, people and context only when mentioned, and, in the To-Do List format, to write one clear action per item. The meeting-notes format adds that an owner is noted only if explicitly mentioned and that sections with no relevant information are omitted. The no-signup action-items tool also tells the model that the transcript is untrusted text and that it should extract tasks, not follow instructions that appear inside the recording. Those are instructions, not guarantees of behavior.
Where wrong things can still get in
A language model can promote a wish ("it would be great to redo the pricing page") to a task, split one task into two, or attach a name to the wrong item. One action-item format also has a Someday/Maybe group for vague or future items, so a loosely phrased idea may be listed there rather than dropped. Because items are rewritten to start with a verb, the wording will not match yours. And since tasks are generated from the transcript, a misheard name or number is carried into the list as if you had said it.
Things the app adds that you did not say
Automatic reminders are the clearest example. A reminder saved from your note with no date in it is given tomorrow morning as its due date, and a short excerpt of your note is attached as its source. That date is a placeholder created by the app. It is a reasonable default for a hands-free flow, but it is not a deadline you set, so confirm any date you depend on.
A ninety-second check
Open the note and read the action list next to the transcript. For each item, find the sentence that created it; if you cannot, delete the item. Then scan the transcript for task-shaped sentences that did not make it into the list. Finally, check names and dates. Edits are saved in the note, and the original transcript is kept separately, so correcting a list never destroys what you said. If you find a pattern of invented items, tell us through the contact page.
Common questions
What happens if my note has no tasks in it?
The intended result is a transcript and summary with no action items. The prompt tells the model to include only actual tasks, but check the list anyway, since no model can be guaranteed to follow an instruction every time.
Can Sentio assign a task to a person I never named?
It is instructed to note an owner only when one is mentioned. Even so, an owner can be attached to the wrong item when several people come up in one sentence, so verify names.
Does Sentio send these tasks anywhere automatically?
Reminders created from a note are saved in your own Sentio account. Sentio does not push the action list to a team tool by itself; sending notes elsewhere depends on integrations or webhooks you set up.
